    Mean spiritedness and superiority aside, it's also just very trite and hackneyed. Spirit not aside, it doesn't even read like a joke, just someone desperate to feel superior and projecting their insecurities.

    Nah, "don't do real work" and "refuse to go to therapy" feel like weird personal attacks, and the follow-up is just restating their opinion sincerely. This person is clearly mad and can't cope.

    That's exactly the point. It has to feel like a personal attack in order to better convey the joke. Many famous commedians construct their jokes that way. I guess that you are just not familiar with that type of humor, which is ok because it really isn't for everyone. Something like comparing Friends with it's always Sunny in Philadelphia - a lot of people just can't stand Sunny jokes because they are overtly abrasive.
